If you or someone you know is battling cancer and would like to apply for our services, please let us know by taking the following steps:

1. Download our New Client Application.
2. Complete our application packet in its entirety.
3. Initial, Date, Sign all pages.
4. Return your completed application packet to Maribo Cares along with a photocopy of your government issue identification card or driver’s license.     *** ALL PERSONAL INFORMATION GIVEN TO MARIBO CARES REMAINS CONFIDENTIAL.
5. Applications may be emailed back to us: info@maribocares.org or sent via the postal system to our address on the application.
